Dermapen is a brand of microneedling pen. It is not a different treatment. Asking whether to have Dermapen or microneedling is like asking whether to have a Bic pen or handwriting. Dermapen is one of several motorised pens a clinic can use to perform microneedling.
So the useful question is not which name to pick. It is what the appointment actually involves: how deep the needles are set, whether the cartridge is single-use and sterile, who is holding the pen and in what kind of facility, and whether radiofrequency heat is being added on top.
The regulatory anchor is a different brand entirely. SkinPen Precision was authorised through a De Novo request, DEN160029, granted 1 March 2018. It was the first microneedling device authorised for marketing in the US, and it created the FDA category these devices sit in - 21 CFR 878.4430, product code QAI. Most other pens, including Dermapen, are ordinary 510(k) clearances that rest on being substantially equivalent to it.
Dermapen 4 has its own clearance, and it is narrower than SkinPen's. The "DP4 microneedling device" was cleared as K221070 on 20 December 2022, "intended to be used as a treatment to improve the appearance of facial acne scars in Fitzpatrick skin types I, II, III, IV and V in adults aged 22 years or older". SkinPen's current wording also covers wrinkles of the neck in Fitzpatrick skin types II to IV. Dermapen 4 has no neck indication - a real difference that clinic pages routinely blur.
Needle depth. Pens are adjustable, and depth is chosen by the operator. Dermapen 4 offers 0.2 to 3.0 mm and SkinPen 0 to 2.5 mm, so two appointments using the same brand can be very different treatments.
Whether the cartridge is single-use and sterile. Needles that enter skin should be sterile and used once. This is the part of microneedling with a real, documented hazard attached to getting it wrong.
The operator and the facility. The clearest published illustration is an outbreak investigation published in MMWR in April 2024. Four former clients of one New Mexico spa, plus the sexual partner of a client, were diagnosed with HIV between 2018 and 2023 despite low reported risk; the clients had received platelet-rich plasma microneedling facials there. Genetic sequencing showed highly similar virus across the cases. The facility was unlicensed, did not follow recommended infection control procedures, and kept no client records. The lesson is not that microneedling transmits HIV. It is that a procedure involving needles and blood needs a licensed facility, sterile single-use equipment and proper record keeping.
Whether radiofrequency is added. Some pens are paired with, or replaced by, radiofrequency microneedling devices, which make the same channels and then release heat at a chosen depth. That is a different treatment with a different risk list, a different price and different paperwork - most radiofrequency devices in this class are cleared only for electrocoagulation and hemostasis, surgical wording for sealing tissue and stopping bleeding, and name no cosmetic use at all.
What gets applied afterwards. A 2025 systematic review of granulomatous reactions to microneedling collected 13 studies and 15 patients with stubborn inflammatory lumps after treatment. Motorised microneedling pens and topical vitamin C applied to the treated skin were implicated in most cases, and improvement was inconsistent across steroids, antibiotics and anti-inflammatories. Ask what is going onto your skin immediately after the needles come out.
It is used for atrophic acne scarring (the sunken, pitted kind), general texture, fine lines and the appearance of pores, and - with SkinPen, in Fitzpatrick skin types II to IV - neck wrinkles.
It does not meaningfully tighten loose skin, and it does not reach or heat the fat beneath, so it has no contouring effect at all. There is no energy involved; the whole effect is injury plus healing.
On evidence: a 2024 review pooled 24 randomised trials and 1,546 participants on microneedling for acne scars and ranked the treatments against each other. Microneedling combined with other treatments outperformed microneedling on its own, and microneedling with a chemical peel ranked best on improvement, satisfaction and efficacy.
Recovery is conventionally described as redness like a moderate sunburn for roughly 24 to 72 hours, with mild swelling and occasional pinpoint bleeding on the day, over a course of about three to six sessions four weeks apart. Those are clinical convention rather than figures from any FDA document.
If you get cold sores, tell your provider when you book, not on the day. Antiviral medicine is started in advance, so it has to be prescribed before the appointment. There is no published data on cold-sore reactivation after microneedling specifically - the precaution is carried over from laser resurfacing, where reactivation is well documented.
Treatment should be deferred if there is any active infection in the area, including an active cold sore, impetigo or cellulitis. Microneedling opens thousands of channels and can spread a local infection across the whole field.
If radiofrequency is being added to your treatment, also disclose a pacemaker or implantable defibrillator and any metal implant near the treatment area. These are labelled contraindications for radiofrequency devices, precautionary rather than tested, and your cardiology team is the right source of advice.
