Urea does different things at different strengths. At low strengths it is a humectant that hydrates. At 40% it becomes a keratolytic — it breaks down and dissolves keratin, the protein in thick, dead skin.
On a thick callus, that is exactly what you want. There is a mass of dead tissue for it to work on. The FDA's own labelling for urea 40% describes its purpose as debriding and helping normal healing of thickened skin, corns, calluses and damaged nails.
On normal skin there is no callus to consume, so it acts on your healthy barrier. That is why people who use it as a general moisturizer find it irritating. It is the right tool for the wrong job when used all over.

For what it is actually for, it works.
40% is the recognised strength for debriding thick, dead skin. The FDA's own labelling for urea 40% states its purpose plainly: "debridement and promotion of normal healing of hyperkeratotic surface lesions" — corns, calluses, thickened heels, and "damaged, ingrown and devitalized nails."
If you have hard, cracked heels or thickened toenails, this is a genuinely effective tool, and it costs very little.
Its real product name is "Urea 40% Foot Cream," and some listings ship it with a pumice stone and a brush — which tells you everything about what it is for.
This is not a body moisturizer, and it is not a face cream. It is a debriding agent.
At 40%, urea does not moisturise — it dissolves keratin. On a thick callus, that is exactly what you want: there is a mass of dead tissue for it to work on.
On normal skin, there is no callus to consume — so it works on your intact barrier instead. Expect stinging, burning, redness and peeling. People buy this as a general moisturizer and cannot understand why it is destroying their skin. This is why.
It contains tea tree oil — a known contact sensitizer — in a product designed for cracked, fissured skin. That is a poor decision.
It is sold under at least three different brand names — PurOrganica, PurSources, Open Naturals — for what appears to be the same product.
