What actually makes a result subtle

"Subtle" is one of the most common things people ask for, and it is a reasonable request. The way to get it is a conversation about dose, not a choice of brand.

Three levers, and your injector controls all of them:

• The dose. Fewer units in a muscle weakens it partly rather than fully. This is the main lever.
• The number of injection points. Fewer points across a broad muscle leaves more of it working.
• Where the points go. Placement can soften the middle of the forehead while leaving the outer parts, which lift the brow, alone.

The brand is not on that list. Jeuveau, Botox, Xeomin and Letybo all use 20 units for frown lines; Dysport uses 50 on its own scale. None of them is inherently the "light" option.

About "baby Botox" and microdosing

There is a real technique behind the marketing phrase, and it is worth understanding what is and is not known about it.

In the published literature it is called microbotox, mesobotox or intradermal injection. The toxin is heavily diluted and placed in many tiny amounts just under the skin surface — very roughly 0.1 to 0.2 units per point, against 0.5 to 2 units for standard injection. The goal is smoother skin texture and softer fine lines without the frozen look.

Two honest caveats:

• The evidence is thin. The technique traces back largely to a single surgeon's case series, and reviews since then describe wide variation in dilution, number of points and depth. It is not a standardised protocol.
• The "it wears off faster" claim is not established. Reviews report a duration of three to six months, which overlaps standard dosing. No study has compared the two head to head. Do not budget for a shorter result on the strength of that claim.

And specifically on Jeuveau: nothing has been published on Jeuveau at reduced doses. Its entire trial programme used the standard 20 units. The only dose variation its maker studied was a higher one.

What to ask for at your appointment

Rather than asking for a brand, describe the outcome:

• "I want to still be able to raise my eyebrows."
• "I want the frown softened, not gone."
• "I would rather be under-treated and come back than be frozen."

Then ask directly:

• How many units are you using, and how does that compare to a standard dose?
• Which movements will I keep?
• If it is too strong, what can we do? (The honest answer is: nothing but wait.)
• If it is too light at two weeks, can we add units, and is that included?

Starting low is the safe direction. Units can be added at a two-week review. They cannot be taken out.

What subtle will not fix

Botulinum toxin relaxes the muscle that folds the skin. Lines that are etched in and visible when your face is completely still will soften a little at best. A lower dose softens them less.

If the lines that bother you are there when you are not moving at all, toxin alone — at any dose — is not the treatment. Ask about resurfacing or filler instead.

Get emergency help now if

• You have trouble swallowing, speaking, or breathing
• You feel weakness spreading to muscles you did not have treated

Go to an emergency department or call 911. Do not wait for a call back. These are rare, but they are the symptoms named in the FDA boxed warning that every botulinum toxin product carries, deaths have been reported, and they can begin hours or even weeks after treatment.

Call your injector if

• One eyelid or eyebrow starts to droop, or your vision changes
• One side sits noticeably higher than the other at two weeks

If a vision change is sudden or severe, seek urgent care rather than waiting.
