Q-Switched Laser

The Q-switched laser is the original pigment and tattoo laser, firing pulses that last billionths of a second. Picosecond lasers have largely replaced it for tattoos, but it is still widely used, still effective, and usually cheaper.

A Q-switched laser delivers very short, very powerful pulses — nanoseconds, or billionths of a second — that shatter pigment without heating much of the skin around it. For decades it was the only way to remove a tattoo, and it still removes tattoos and brown spots well. Newer pico lasers use pulses about a thousand times shorter and generally clear ink in fewer sessions with slightly less pigment risk. Q-switched machines remain common, cost less per session, and in trained hands do most of the same jobs.

How It Works

"Q-switching" is a trick of laser engineering. A shutter inside the laser holds energy back while it builds up, then releases it all at once. The result is a pulse that lasts only nanoseconds but carries enormous peak power.

That speed is the point. Tattoo ink and melanin absorb the light and heat instantly. Because the pulse ends before the heat can spread to neighboring tissue, the particle fractures rather than the skin burning. Dermatologists call this selective photothermolysis: destroy the target, spare the surroundings. Your immune system then clears the fragments over weeks, which is why tattoo removal takes months rather than one visit.

A picosecond laser takes the same idea further. Its pulses are roughly a thousand times shorter, so more of the energy goes into mechanical shattering and less into heat, producing finer fragments and fewer sessions. The Q-switched laser is the older, slower version of the same physics — not a different or discredited technology, just less efficient.

Q-switched machines come in several wavelengths, and the wavelength determines what the beam can target.

Q-switched lasers for periorbital hyperpigmentation have inconsistent results and a meaningful rate of post-inflammatory hyperpigmentation in skin of colour. Conservative settings and test spots are standard.

Who it's right for

People with black or dark blue tattoos, which respond best of all.

Flat brown spots, lentigines and freckles on the face, chest and hands.

Dermal birthmarks including nevus of Ota, where Q-switched lasers have a long track record.

People with brown or Black skin needing pigment treated, using the 1064 nanometer wavelength at low fluence. This is a well-established approach and one of the reasons Q-switched Nd:YAG machines have stayed in clinics.

Anyone weighing cost. A Q-switched course usually costs less per session than a pico course, even if it takes more visits.

Who it's not for

Anyone with a tan or a self-tan on the area.

Anyone hoping for wrinkle or scar resurfacing. This laser targets pigment, not texture. Fractional and ablative lasers do that job.

Anyone with a large multicolored tattoo who wants the shortest possible course. A picosecond laser will usually get there in fewer visits.

Anyone with an active infection, broken skin, or a recent isotretinoin course in the treatment area.

People with melasma hoping for a cure. Low-fluence 1064 nm treatment lightens it temporarily and it relapses; overdone, it causes pale speckling that is difficult to correct.

How to Prepare

Before you start

Ask which wavelength will be used and why. For black ink or for deeper skin tones, the answer should involve 1064 nm.

Ask for a test spot if you have brown or Black skin, and have it read two to four weeks later.

Photograph your tattoo or spots yourself in consistent light. Progress across a long course is hard to judge from memory.

Ask what the realistic total number of sessions is for your tattoo, and what the total will cost. Q-switched courses are longer than pico courses.

In the two weeks before

No sun, sunbeds or self-tanner on the area.

Pause retinoids and exfoliating acids on the area for several days.

Antiviral tablets from the day before if you get cold sores and the area is near the mouth.

On the day

Clean, bare skin over the treatment area.

Numbing cream 30 to 60 minutes ahead for tattoo work. A few spots usually needs none.

What Happens During It

Everyone in the room wears protective eyewear.

The provider fires a few test pulses at an edge and watches the skin before treating the rest.

Each pulse feels like a hot rubber band snap or a splash of grease. Tattoos are more painful than spots, and bony areas more painful than fleshy ones.

Treated pigment usually frosts white within a second. That is gas escaping from shattered particles and it fades in 15 to 20 minutes.

A few spots takes a couple of minutes. A hand-sized tattoo takes 10 to 20. Big pieces get split across appointments.

Cold air or ice is used during and afterwards. Tattoo areas get ointment and a dressing before you leave.

You go home red, swollen and warm, and tattoo areas often look welted.

Recovery

Practical rules: gentle washing, keep it moisturized, no pools, hot tubs or heavy sweating until scabs are gone, and daily sunscreen on treated skin for at least a month.

Day 0
Redness, heat and swelling for a few hours. Frosting fades within the hour. Tattoo sites may weep.
Days 1-3
Brown spots turn dark, sometimes almost black, and feel rough. Tattoo sites may blister. Blisters are more common with Q-switched lasers than with picosecond ones because more heat is delivered. Keep them intact, covered with ointment and a dressing.
Days 4-14
Darkened spots flake off. Tattoo scabs lift. Let everything separate on its own; picking causes the pale marks and scars people blame on the laser.
Weeks 2-8
Skin looks normal, and ink continues clearing invisibly. Sessions are spaced six to eight weeks apart for that reason.

Side Effects & Risks

Common and expected
  • Redness, swelling and warmth for a few hours to two days

  • A blister that is large, or any blister on the face
  • A firm raised ridge developing in the treated area
Rare but serious
  • Spreading redness, increasing pain, pus or yellow crusting after a few days — infection
  • A broken blister leaving a raw area that is not closing over several days

Results

Tattoos
Black and dark blue ink fades reliably. Complete or near-complete clearance is achievable, typically over six to fifteen sessions — more than a pico laser needs, but at a lower price per visit. Amateur tattoos clear faster than professional work. Green, light blue and yellow are stubborn, and a faint ghost of a dense tattoo is a common endpoint. White, pink and flesh-toned cosmetic inks can turn permanently black on the first pulse.
Sun spots, lentigines and freckles
Usually clear in one to three sessions and stay gone with sun protection. New spots keep appearing if you keep getting sun; the laser treats the marks, not the cause. See sun damage and hyperpigmentation.
Nevus of Ota and dermal pigment
Good long-term outcomes are well established, but it takes a course of many sessions.
Melasma
Low-fluence 1064 nm toning lightens it during a course and relapse is the norm. Pushed too hard or too often, it can cause a mottled loss of pigment. Topicals such as hydroquinone and tranexamic acid, strict sun protection and sometimes oral tranexamic acid do more of the work and carry less downside.
What it does not do
Wrinkles, pores, laxity and deep acne scarring are not Q-switched laser problems. If a clinic offers you a Q-switched laser for those, that is marketing, not medicine.

At-Home Versions

There is no home Q-switched laser. The peak powers involved are far beyond anything sold to consumers.

Handheld "picosecond pens" and blue-light tattoo removal devices sold online are not Q-switched lasers, do not remove tattoos, and can burn skin and damage eyes. Tattoo removal creams cannot reach ink held in the dermis; anything strong enough to reach it would scar you.

For flat brown spots, an at-home routine — daily sunscreen, a retinoid, and a lightening agent such as azelaic acid, tranexamic acid or alpha arbutin — is slower but genuinely useful and prevents new spots.

FAQ+
Is a Q-switched laser obsolete?No. It is older technology and generally needs more sessions than a pico laser for a tattoo, but it removes ink and brown spots effectively and costs less per visit. For a small black tattoo or a few sun spots, the practical difference between the two is small.
How many sessions to remove a tattoo?Usually six to fifteen at six to eight week intervals. Dense, colorful, large or previously treated tattoos take more. Amateur tattoos take fewer.
Which is safer on brown or Black skin, Q-switched or pico?Pico has a small edge, because a shorter pulse spreads less heat.
