Fat cells die when they are held above roughly 42 degrees Celsius for long enough. Skin does not tolerate that temperature. SculpSure gets around the problem by choosing a wavelength that fat absorbs more readily than skin does, and by cooling the surface at the same time.
The laser runs at 1060 nanometers. Light at that wavelength passes through the top layers of skin and is preferentially taken up by the fat underneath, so the fat heats faster than the tissue above it. The applicator face is cooled throughout, which keeps the skin surface in a safe range while the fat below climbs into the damaging zone. The laser also cycles on and off, letting the skin shed heat between pulses.
Fat cells heated in this way do not disappear on the table. They are injured, then die over the following weeks, and immune cells clear the debris. The visible change follows that clearance, which is why nothing happens for the first month.
Note what this shares with fat freezing: the amount of fat lost is a fraction of what is in the treated patch, the fat cells that survive can still enlarge if you gain weight, and none of it reaches the fat around your organs. The difference between heating and freezing is mostly how it feels and what shape of applicator can be used, not what the treatment can achieve.
Ask to be assessed lying down and standing. The applicators sit flat, so the provider needs to see how the area behaves in both positions.
Get the plan and the total price, not the per-applicator price. Two or three rounds per area is the normal course, and a single session is rarely the finished job.
Tell the provider about tattoos, implants, recent surgery in the area, and any medicine that makes your skin light-sensitive.
Have your weight steady before you start. A moving baseline makes the result impossible to read.
Eat normally. There is no sedation and no fasting.
Skip heavy moisturizer or oil on the area — the panels need clean contact.
Plan for about half an hour of lying still, plus set-up.
The provider marks where the applicators will go and fastens the belt around you, holding the flat panels against the skin.
The cycle starts with about four minutes of cooling only. The panel feels cold.
Then the laser begins to cycle. You feel alternating waves of deep heat and cooling. The heat builds through the session and most people describe it as a hot, prickly or tingling sensation, occasionally sharp for a few seconds.
There is a control in your hand or a way to signal the provider. If the heat becomes too much, the operator turns it down. Being too stoic here is not useful — the target is uncomfortable but tolerable.
The cycle runs 25 minutes. You lie still. There is no vacuum and no pulling.
At the end the panels come off. The skin underneath is pink and warm. Unlike fat freezing there is no frozen block of tissue and no vigorous massage afterwards.
You get dressed and leave.
You go straight back to normal activity, including exercise the same day.
The first day: mild redness and warmth in the treated rectangles, sometimes a little swelling.
The first one to three weeks: the most common leftover is tender, firm lumps or nodules under the skin in the treated area. They can be surprising to find and they are expected — inflamed fat clearing. They soften over a few weeks.
Tingling, tightness, itching and occasional twinges of sharp pain in the area are common in the first couple of weeks.
Bruising is uncommon, because there is no suction.
Numbness is much less of a feature than after fat freezing. Some people get none at all.
If a lump is still firm and tender after about eight weeks, mention it. It usually still settles, but the clinic should have it recorded.
What you get is a thinner fat layer under the panels — around a fifth to a quarter of the fat in the treated patch per round of treatment. That reads as a flatter contour and a slightly better fit in clothes, not a transformed silhouette.
It does not change your weight, and the scale generally does not move.
Timeline: nothing at three weeks, something at six, the real answer at twelve. The lumpy phase in the first month is not the result.
Who is happy: people with a flat, broad area of soft fat, near a stable weight, who wanted the area to sit smoother.
Who is disappointed: anyone expecting liposuction from a 25-minute laser session; anyone with loose skin, which this does not tighten; anyone with a large volume of fat, where the proportional change is too small to see; and anyone who gains weight afterwards, since the surviving fat cells will enlarge.
Durability: the fat cells destroyed are gone for good. The look holds if your weight does.
Against the alternatives: results from SculpSure, cryolipolysis and radiofrequency fat reduction land in a similar range, and no honest comparison puts a large gap between them. Liposuction removes far more, in one go, with anesthesia and real recovery. For fat under the chin specifically, Kybella is an injected option with more swelling and a similar order of change. If what bothers you is skin laxity rather than fat, Thermage or Ultherapy address a different problem and remove no fat.
There is no home version, and there is no plausible one.
The whole treatment depends on holding fat above a damaging temperature while keeping the skin below a damaging one. That requires a specific wavelength, a cooled applicator face, and monitoring. Heat pads, sauna belts, infrared wraps and "fat burning" body wraps do none of that. They warm the surface, which is the layer you need to protect, and they do not reach or hold the fat at a temperature that matters.
The temporary inch loss people report from wraps is water loss from sweating, and it returns with your next drink.
Nothing you can buy will target fat in one chosen spot. Whole-body approaches — eating and activity — are what change body fat at home, and they act everywhere rather than where you point them.
