Ultraformer focuses ultrasound so that the waves converge at a single point below the skin. At that point the tissue reaches roughly 60 to 70 degrees Celsius and coagulates; the tissue the wave passed through on the way stays cool. Thousands of these dot-sized injuries are placed across the treatment area, and over the following months the body replaces each one with new collagen. That is the mechanism shared by every device in this family.
The energy is delivered through interchangeable cartridges, each fixed at one depth. Shallow cartridges work in the dermis and improve skin quality; deeper ones aim at the fibrous layer beneath, which is where lifting comes from. Body cartridges reach considerably deeper again and are used on the abdomen, flanks and arms, where the target includes fat as well as connective tissue. The newer MPT version adds a mode that moves the energy delivery continuously rather than firing discrete lines, which is intended to be faster and more comfortable.
The important structural point is that there is no imaging. The operator picks a cartridge depth from a treatment protocol and places it on the skin. Whether 4.5 millimeters on your particular temple is fibrous tissue, fat or periosteum is not something the machine can show them. Ultherapy can. That is the real technical difference between the two, and it matters more on thin, bony areas than on a full cheek.
Whether it matters enough to justify the price gap is a fair question with no clean answer. Experienced operators treat safely on unimaged devices all the time, using anatomy and conservative depth selection. But when something goes wrong in this category, it is usually because energy landed somewhere nobody could see.
Ask which model, which cartridges, and how many shots. All three are reasonable questions and a good clinic answers them without hesitation.
Ask about the regulatory status where you are being treated. In the US, ask specifically whether this device is FDA-cleared for what you are having done.
Tell the provider about filler, threads, implants, and any electronic implanted device.
Have proper before photographs taken. The change is slow enough that memory will not serve you.
Come with a clean face.
Discuss pain relief in advance. Numbing cream is commonly used with this device and it needs 30 to 45 minutes to work, so arrive early if you want it.
Eat something first. Sessions typically run 45 to 90 minutes.
The area is cleaned, mapped into treatment zones and covered in ultrasound gel.
The provider selects a cartridge and works line by line across each zone, firing as they go.
Each line is a second or two. The sensation is a brief deep heat or an internal prickle. Over bone — jawline, brow ridge, forehead — it is sharper. The continuous-motion mode on the newer platform is generally described as gentler than discrete firing.
Cartridges are changed to treat at more than one depth. If only one cartridge is used on the whole face, you are getting one layer of what is normally a layered treatment.
Face and neck usually takes 45 to 90 minutes. Body areas take longer.
Afterwards the skin is flushed and often slightly swollen, sometimes with faint temporary lines where the energy went in.
No downtime. Normal activity straight away.
Redness fades over a few hours.
Swelling for one to three days, most obvious under the chin and jaw.
Tenderness on pressing the area for up to a couple of weeks. Chewing may feel slightly uncomfortable for a few days.

Nothing visible changes for the first month or more. That is the expected course, not a failed treatment.

Duration is commonly around a year to eighteen months before it fades back into ongoing ageing. Many people repeat annually.
On the body, results are less predictable. Abdominal and arm laxity respond less reliably than facial skin, and expectations should be set low.
Who is disappointed: people who had significant sagging. This is the recurring honesty problem across every device on this list — focused ultrasound is most effective in people who barely need it and least effective in the people most motivated to try it. If you can pinch a fold of loose skin, this is not the answer and no number of sessions will make it one.
The other disappointment specific to this device is inconsistency between clinics. Because dose is set by shot count and cartridge choice, and because cartridge condition is invisible to you, two treatments sold under the same name can differ enormously. That is less true of platforms with tighter distribution and mandatory consumables tracking.
Compared with Ultherapy: same category, same order of effect when well delivered, meaningfully cheaper, and without the ability to see the tissue before firing. Compared with Thermage: different energy, similar magnitude, radiofrequency being a broader heat and ultrasound a more targeted one.
There is none, and there should not be. Focused ultrasound heats tissue to coagulation temperature several millimeters below a surface you cannot see through, near nerves. That does not belong in a consumer device.
Handheld ultrasound skincare tools sold for home use emit low-intensity, unfocused ultrasound. It warms the surface and helps products spread. It is a different technology with a similar-sounding name, not a weaker version of this treatment.
The at-home approach that genuinely improves skin firmness over months is unglamorous: a retinoid, daily sunscreen, and time.
