Microneedling vs RF Microneedling: Is the Heat Worth the Extra Cost?

Microneedling has been around long enough to be routine. RF microneedling — sold as Morpheus8, Vivace, Genius and others — is the newer, pricier version, and clinics push it hard.

The devices look similar and the needles are the same idea. The difference is that RF versions send radiofrequency energy out of the needle tips once they are in the skin, adding controlled heat to the controlled injury.

The question is whether that heat is worth the extra money for your particular concern. Sometimes clearly yes. Often no.

Both live almost entirely in the dermis row — this is collagen work, not surface skincare. The needling is identical; the only real variable is heat. Heat is what extends RF microneedling's reach into tightening the collagen already there, and it is also what lengthens the barrier recovery and raises the (still low) pigment risk. Neither one touches pore chemistry, inflammation, or antioxidant defense.

When to choose which

Rolling and shallow acne scars | Either | Moderate
Plain microneedling does well here. A systematic review of nine randomized trials found it consistently effective for indented acne scars, alone or combined with other treatments, with no serious side effects reported (Int Wound J, 2021).

Deep, boxcar, or tethered scars | RF microneedling | Moderate
Heat reaches deeper and does more remodelling. In a split-face trial, a single RF microneedling treatment matched ablative fractional CO2 laser — the long-standing benchmark for scars — on texture improvement at 3 months (Lasers Surg Med, 2023). Matching CO2 laser is a meaningful result.

Loose skin, jowls, jawline, neck | RF microneedling | Moderate
Plain microneedling does not tighten. Heat does. This is the clearest reason to pay more.

One to three days of redness, versus two to five days plus visible grid marks. Against ablative laser, though, RF microneedling was the gentler option — less redness and less barrier disruption at 2–4 days (Lasers Surg Med, 2023).

The heat hurts. In that same trial, patients rated RF microneedling as significantly more painful than the CO2 laser, 7.0 versus 5.5 on a 10-point scale. Numbing cream is standard for both, but RF is the less pleasant hour.

Both are considered safe, and both far safer than ablative lasers. Plain microneedling adds no heat at all, which removes the main pigment risk entirely. RF microneedling is generally well tolerated because the energy is delivered below the pigment-producing layer — but "generally" is doing some work there, and operator skill matters more.

Can you use both?

These are not a stack — pick the one that matches your concern. Doing both in the same session is not a thing.

Choose plain microneedling if: your concern is texture, pores, shallow rolling scars, or early lines; your budget is limited; or you want the shortest downtime.

Choose RF microneedling if: you have loose skin, deep or tethered scars, or jawline and neck concerns — anything where the goal is tightening, not just smoothing.

People do sometimes use them in sequence, with a course of RF microneedling for the structural work and plain microneedling for maintenance afterward. That is a reasonable plan, not a required one.

What to be honest about: these two have rarely been compared directly to each other in randomized trials. Most of the good RF microneedling evidence compares it to lasers. So while the reasoning behind the heat is sound and the laser comparisons are encouraging, a claim like "RF is 40% better than regular microneedling" is not something the research currently supports.

Whichever you choose, results come from the series, not the session — and both need sunscreen afterward.
