Dermatologist's Take

Topical clindamycin is a common prescription antibiotic for acne that works best paired with benzoyl peroxide to prevent resistance. It reduces inflamed pimples but does little for blackheads and should not be used alone long-term.

At a glance

  • AKA: Cleocin T, Clindagel, Evoclin; combos: Duac, BenzaClin, Onexton (generic: clindamycin phosphate)
  • Drug Class: Topical antibiotic (lincosamide)
  • Rx or OTC: Prescription only
  • Typical Strength: 1% gel, lotion, foam, or solution
  • Time to Work: Some change in a few weeks; full benefit by about 8 to 12 weeks
  • Evidence Level: Good for acne, especially when combined with benzoyl peroxide
  • Important: Used alone, acne bacteria can grow resistant. Doctors almost always pair it with benzoyl peroxide.

How it works

Clindamycin slows the growth of Cutibacterium acnes, the bacteria that add to acne. It also calms some inflammation (skin swelling and irritation). Fewer bacteria and less inflammation mean fewer pimples. Benzoyl peroxide is added because it also fights the bacteria and helps stop resistance.

What it treats

How it's typically used

Clindamycin is usually applied in a thin layer once or twice a day to clean, dry skin. It is often prescribed as a combo product with benzoyl peroxide, or used alongside one. Doctors avoid using it alone for long periods. This is educational only — follow your prescriber's directions.

Common side effects

  • Dryness or peeling
  • Redness or irritation
  • Burning or stinging
  • Oily or, at times, itchy skin

Serious side effects

  • Ongoing or bloody diarrhea, or stomach cramps — this can signal a bowel problem called colitis and needs prompt care (rare with skin use)
  • Strong allergic reaction (swelling, hives, trouble breathing)

Call your doctor right away if you have bad diarrhea or an allergic reaction.

Who shouldn't take it

Do not use if you are allergic to clindamycin or lincomycin. Tell your doctor if you have had colitis or bowel disease.
