Focused ultrasound works by converging sound waves so that they all arrive at the same point and heat it, while the tissue the waves passed through stays cool. That much Sofwave shares with the other ultrasound tightening devices.
Where it differs is arrangement and depth. Sofwave's handpiece holds seven small transducers in a line, and they fire together. Instead of a string of separate pinpoint injuries at a chosen depth, the result is a wide, continuous band of controlled heat at a fixed level in the upper dermis — around 1.5 millimeters below the surface. The company calls this SUPERB, for synchronous ultrasound parallel beam.
That heat denatures collagen in the treated band. The repair response over the following weeks lays down new collagen and elastin, which is what firms the skin.
A cooling plate on the handpiece chills the epidermis continuously during treatment, which is what allows a fixed shallow depth to be used safely. Get the cooling wrong and 1.5 millimeters is close enough to the surface to matter.
The design tradeoff is deliberate and worth understanding. Ultherapy can place energy at about 4.5 millimeters, in the fibrous SMAS layer that a surgeon tightens during a facelift, which is where genuine lifting comes from. Sofwave does not go there. It stays in the dermis. That makes it faster, considerably more comfortable, safer around bone and nerve, and less dependent on the operator's skill at reading depth — because there is no depth decision to make. It also means it is fundamentally a skin-quality treatment rather than a lifting treatment, whatever the marketing says about lift.
Be clear with the provider about whether your issue is loose tissue or poor skin quality. If it is loose tissue, ask them directly whether a deeper device or a surgical consultation would serve you better. A good provider will say so.
Ask how many passes and how much of the face is included in the quoted price. Coverage varies between clinics.
Get before photographs taken properly. Three months later you will not remember accurately.
Come without makeup or expect it to be removed.
Numbing cream is optional and often not used, since most people tolerate Sofwave without it. Ask in advance if you want it — it needs time to work.
Nothing else is required. There is no pre-treatment skincare washout and no restriction on activity before or after.
The area is cleaned and ultrasound gel is applied.
The handpiece is placed flat against the skin. Full contact matters — the seven transducers all need to be coupled to the skin for the energy to land evenly, which is why the provider works methodically in small squares rather than gliding.
Each placement is a short pulse. You feel heat, sometimes a quick prickle or snap. Over bony areas — the jawline, the brow ridge — it is sharper. Most people describe it as easily tolerable and notably milder than the deeper ultrasound treatments.
The provider works through a mapped grid so the area is covered without stacking pulses in one spot.
Full face takes about 30 to 45 minutes. Face and neck closer to an hour. A single small area can be 15 to 20 minutes.
You will look pink afterwards, sometimes with faint impressions of the handpiece. Both settle quickly.
There is no recovery period. People go back to work, the gym, or a dinner the same day.
Redness for a few hours is the usual course. Occasionally it lingers into the next morning.
Mild swelling, most often under the chin and along the jaw, for a day or so.
Tenderness when pressing the treated area for a few days. Less than with the deeper ultrasound devices, but not nothing.
Small bruises are possible, particularly on the neck.
Makeup, exercise, sun exposure with normal sunscreen — all fine immediately.
For the first month you will probably see nothing at all. That is the expected course.
What you get is firmer, smoother, less crepey skin. Fine lines soften. The jawline can look slightly cleaner. Under-chin skin tightens a little.
What you do not get is a lift you could demonstrate to someone. The FDA clearance includes lifting the eyebrow, and measured against a ruler the brow does rise slightly. In the mirror, that is a fresher look rather than a changed one.
Timing: nothing for the first month, gradual change from about six weeks, and the peak around three months. Some people have a second session at that point.
Duration: commonly nine to eighteen months before it fades back, then an annual maintenance session if you want to hold it. Nobody should be told this is permanent.
Who is disappointed: people who had significant laxity and were sold a lift. That group would have been disappointed by any device in this category, but Sofwave's shallow working depth makes it a particularly poor match for real sagging. Also disappointed: people who compare the before-and-after photographs on clinic websites with their own result. Those photographs are frequently taken in different lighting.
Compared with the deeper ultrasound devices, Sofwave is quicker, more comfortable, and less operator-dependent, and it produces less lift. Compared with RF microneedling, it has no needles and no downtime, and RF microneedling generally does more for texture and acne scarring. On acne scars specifically, Sofwave is cleared and it does help some people, but resurfacing lasers and subcision remain the stronger options for anything beyond mild scarring.
None exists. Sofwave is a clinic-only ultrasound platform and the transducer array is not something that scales down to a consumer device.
Home ultrasound skincare wands are a different technology altogether. They use low-intensity, unfocused ultrasound that spreads out rather than converging on a hot point. It warms the surface and helps product absorb. It does not remodel dermal collagen and it is not a weaker version of this treatment.
The genuinely effective at-home approach to skin firmness has not changed: a retinoid used consistently, daily sunscreen, and patience. It costs a fraction of a device and has more evidence behind it.
