The protocol is four passes with two wavelengths, and it is easier to understand as four separate small treatments than as one thing.
Step one, called SmoothLiftin. An erbium:YAG laser at 2940 nanometers applied intraorally — the handpiece goes inside your mouth and the energy is delivered through the inner cheek and lip mucosa, aimed outward at the tissue underlying the nasolabial folds and jowls. Erbium at 2940 is very strongly absorbed by water, so at low, non-ablative settings it heats the surface it touches. The theory is that heating the tissue from the inside stimulates collagen where it supports the folds. This is the step that makes Fotona 4D distinctive, and it is also the step with the least independent evidence behind it.
Step two, FRAC3. Nd:YAG at 1064 nanometers, delivered externally in short pulses. At this wavelength the light travels deep and is absorbed most by areas that differ from surrounding tissue — pigment clusters, vessels, damaged spots — creating a self-organizing pattern of deeper heating without treating the surface uniformly.
Step three, PIANO. The same 1064 wavelength, but delivered as a long, slow, continuous heating pass. The goal is bulk heating of the dermis to a temperature that provokes collagen contraction and new collagen formation, without hot spots. This is the same principle as other bulk-heating tightening devices.
Step four, SupErficial. Erbium at 2940 again, this time as a very light surface pass — effectively a gentle laser peel that improves texture and brightness.
Two things are worth saying plainly. First, none of these four steps is unique technology; they are settings on a laser that many manufacturers offer variations of. What Fotona owns is the sequence and the name. Second, all four are non-ablative or very superficial, which is why there is no downtime — and why the results are correspondingly gentle.
Ask directly what result is realistic for your face, and ask to see the provider's own before-and-afters rather than Fotona's marketing images. This is a subtle treatment and the marketing photos for it are unusually flattering.
Ask whether you are being sold 4D or 6D, and what the extra passes are.
Stop retinoids and acids about five days before.
No tanning or significant sun for two weeks.
Antiviral medication starting the day before if you get cold sores. The intraoral step and the perioral treatment are both cold sore triggers, and this one matters more than usual.
Deal with any dental problem first. An abscess, an ulcer or active gum inflammation should be treated before anyone points a laser at the inside of your cheek.
Clean face, no makeup.
Brush your teeth. The first step goes in your mouth.
Numbing cream is often used for the external passes but is not always necessary — the whole protocol is designed to be comfortable.
Allow about an hour to ninety minutes. Four passes takes time.
No special aftercare supplies needed beyond your usual moisturizer and sunscreen.
Photos and cleansing. Numbing cream on the face if the provider uses it.
Eye protection throughout.
The intraoral pass first. A small handpiece is placed inside your mouth, against the inner cheek and around the lip area, and moved slowly. It feels warm, building to hot, and most people find this the strangest part of the appointment rather than the most painful. Your mouth is held open with a retractor or by you. It takes several minutes per side and the warmth lingers. Some people find the heat inside the mouth genuinely unpleasant; it is worth knowing this is coming.
The external Nd:YAG passes. The handpiece glides over the face. FRAC3 feels like scattered warm snaps. PIANO is a slow, steady building heat, like a warm iron passing repeatedly over the skin — providers usually work to a temperature they measure with an infrared thermometer, and you will be asked to say when it gets too hot. Say so. That feedback is the safety mechanism.
The final erbium pass. Quick, light, a warm tingling across the surface.
The whole sequence takes 45 to 75 minutes.
Afterwards the face is pink and warm, sometimes with mild swelling. Most people go straight back to their day.
There is essentially no downtime, which is the protocol's main practical selling point.
Redness and warmth for a few hours, occasionally into the next morning. Some people have mild puffiness.
The inside of the mouth may feel slightly tender or dry for a day. Avoid very hot or spicy food that evening.
Skin can feel tight in a pleasant way for a few days. Some people notice a short-lived firming immediately, which is heat-driven tissue contraction and mostly settles within a week. The lasting change comes later and more quietly.
Occasionally there is very light flaking a day or two after the final erbium pass. A bland moisturizer handles it.
Sunscreen daily as usual. Skip saunas and heavy exercise for 24 hours. Resume retinoids after two or three days.
What people actually get: skin that looks a bit fresher and feels firmer, some softening of fine lines, slightly better texture and a modest improvement in the nasolabial area over a course of three. Most people are quietly pleased and nobody comments.
What people are promised: a non-surgical facelift. That framing is the problem with this treatment. Nothing in the protocol lifts tissue, repositions fat, or tightens the underlying layer that a facelift addresses. The word "facelift" in the marketing sets an expectation that the results cannot meet, and a lot of the disappointment with Fotona 4D comes from that gap rather than from the treatment doing nothing.
Timeline: an immediate tightened feeling that is largely temporary, then real collagen change building over three to six months. Judge it at four months after the last session, not at one week.
How long it lasts: roughly a year for most people, with twice-yearly maintenance sessions commonly recommended. That is an ongoing cost of a few thousand dollars a year, worth calculating before you start.
The evidence position, honestly: the underlying laser modes are well established individually. The specific four-step protocol has very little independent published evidence, and almost nothing comparing it against doing one or two of the steps alone. Whether the intraoral pass contributes meaningfully to the result, or is a memorable piece of theater that justifies a premium price, is a fair question that the published literature does not currently answer.
Who is disappointed: anyone with real laxity who bought a facelift comparison, anyone expecting visible change after one session, and anyone who could have got more from a single RF microneedling session or a properly placed filler for the same money.
There is no home version. Medical lasers of this class are not sold to consumers, and the intraoral step in particular is not something anyone should attempt to approximate.
At-home radiofrequency and LED devices are sometimes sold as a tightening alternative. They deliver a small fraction of the energy and produce, at best, a subtle temporary effect. If you enjoy using one, it is harmless. It is not a substitute.
The realistic home contribution to what Fotona 4D aims at is the same as for any collagen treatment: a nightly retinoid such as tretinoin or over-the-counter retinol, daily sunscreen, and vitamin C in the morning. That routine genuinely builds collagen over years, costs very little, and does more for long-term skin quality than an annual protocol does on its own.
If the goal is the nasolabial folds specifically, it is worth knowing that a small amount of dermal filler placed well does more for that fold in twenty minutes than a course of laser tightening will. Whether you want filler is a separate question, but the comparison should be made honestly before you spend three thousand dollars on the laser route.
