Tixel

Tixel has no light and no electricity. It touches your skin with a hot titanium tip for a few thousandths of a second and transfers heat directly, the way a clothes iron does. It is the simplest mechanism in aesthetic resurfacing and one of the least understood.

How It Works

Almost every device in aesthetic dermatology delivers energy that something in the skin absorbs. Lasers deliver photons absorbed by water, melanin, hemoglobin or sebum. Radiofrequency delivers current that meets electrical resistance. Ultrasound delivers mechanical vibration. Tixel does none of that. It heats a piece of metal and touches you with it.

The tip carries an array of small titanium pyramids — the standard configuration is 81 of them — held at about 400 degrees Celsius. The device pushes that array against the skin for a set contact time, typically between five and eighteen milliseconds, then withdraws. Heat moves from metal into tissue by simple conduction, exactly as it would from a hot pan. Each pyramid leaves a small treated column, and the untouched skin between them supplies the cells that heal it. The treated area covers only a small percentage of the surface, which is why healing is quick.

Two settings control everything. Pulse duration sets how long the metal stays in contact, and protrusion sets how far the pyramids push into the skin, adjustable from about 100 to 1000 micrometers. Short contact and shallow protrusion open microscopic channels in the outer layer without much heat below — that is the setting used for drug delivery, where the point is simply to let a topical medicine through. Long contact and deep protrusion produce genuine coagulation columns a couple of hundred micrometers into the skin, which is the setting used for wrinkles and scars.

The consequence of having no chromophore is worth stating plainly: the device cannot tell what color your skin is, cannot see a blood vessel, and cannot target a sun spot. That makes it theoretically usable on any skin tone. It also means it will not clear pigment, because it has no way to find it.

What It Treats

Moderate evidence
Heat moves from hot metal into skin by plain conduction — no light, no radiofrequency, no target in the tissue. So the whole photoaging effect comes from fractional injury and the repair that follows, meaning texture and collagen rather than color. Having no target is also why it can be used across skin tones.
Limited evidence
Because the device is not selective, it treats every skin color the same way. That is useful for safety and unhelpful for clearing color. Studies in pigment problems are small and mixed, so a device that actually targets melanin is the more direct choice when pigment is the main complaint.

For scars the device is set to longer contact time and deeper protrusion, producing coagulation columns a couple of hundred micrometers into the skin. That is enough to remodel mild texture, not to reshape a deep depression. Published data are limited and results vary from person to person.

Who it's right for

People who want fine-line and texture improvement with one to three days of downtime rather than a week.

People with crepey skin and fine lines around the eyes. Tixel is used closer to the lash line than most energy devices, because the tip does not emit anything that could reach the eye, and this is one of its genuine advantages.

People with darker skin tones who want resurfacing without a device that targets melanin.

People who cannot tolerate the pain of laser resurfacing. Reported pain scores for Tixel are low and most treatments are done without numbing.

People needing better absorption of a topical medicine — this is a real medical use, and the drug delivery application is arguably where Tixel is most distinctive.

People with mild scarring, as part of a series.

Who it's not for

Anyone with significant acne scarring hoping for a single solution. Tixel is too gentle at typical settings. Real scarring needs subcision, deep RF microneedling or ablative resurfacing.

People whose main concern is sun spots or pigment. Nothing about this device targets pigment.

People wanting skin tightening or lifting. It works in the upper skin and does not reach the tissue that produces laxity.

Anyone with active infection, an active cold sore, or open acne in the treatment area.

Anyone within six months of finishing isotretinoin, by most practices' policy.

People who need strong evidence before spending money. The published literature is largely small case series, and there is very little comparing Tixel head-to-head with the lasers it is positioned against.

Pregnancy: no data, no urgency. Most practices wait.

How to Prepare

Before you start

Ask what pulse duration and protrusion are planned. With this device those two numbers are the treatment, and they vary enormously between a glow session and a real resurfacing session.

Stop retinoids and acids about five days before.

Antiviral medication starting the day before if you get cold sores, for any facial treatment.

Avoid meaningful sun for two weeks. Not because the device targets pigment, but because stressed skin heals unpredictably.

If your skin tone is deep and the plan involves aggressive settings, ask for a test area and for conservative settings on the first session. The reported pigment problems with Tixel have come from the aggressive end of the range.

On the day

Clean skin, no makeup.

Numbing cream is often not used, because most people tolerate the treatment without it. If you are anxious, ask — it is not a problem to use it, it just adds time.

Plan one to three days of looking pink and slightly gridded, depending on the setting.

Have a bland moisturizer and mineral sunscreen at home.

What Happens During It

Photos and a cleanse. Numbing cream if you have asked for it or if the plan is aggressive.

The device is held against the skin and stamped section by section, in a grid, with the tip making brief contact each time. It is a stamping motion rather than a glide, and you can feel each placement.

Each contact feels like a brief hot press — a quick sting that stops immediately. Most people rate it as mild, and pain scores in the published case series have been low. This is one of the more comfortable resurfacing treatments available, and it is a real selling point rather than a marketing one.

A full face takes roughly ten to twenty minutes.

Afterwards the skin is pink and warm, with a faint grid pattern of small dots. Sometimes mild swelling. A bland moisturizer goes on before you leave.

Recovery

Day one. Pink, warm and slightly tight, with a faint gridded pattern of small dots. Some swelling, particularly if the eye area was treated. Bland moisturizer as often as you want. Nothing active.

Day two. Redness settles. The dots may darken slightly and the skin feels a bit rough. Some people are already presentable with makeup.

Days two to four. Fine flaking, more like dry skin than a peel. Do not exfoliate or scrub it off.

Day three or four onward. Most people look normal. At the lightest settings this stage arrives on day one or two.

Throughout: mineral sunscreen daily, gentle cleanser, no actives. Skip saunas, steam and hard workouts for 48 hours. Restart retinoids at about five to seven days.

If the eyelids were treated, expect a little more puffiness on the morning after than elsewhere. Sleeping propped up helps.

Results

What it does reasonably well: fine lines, particularly around the eyes; surface texture; overall skin quality and brightness. The periorbital area is where Tixel's results are most consistently reported, helped by being able to treat closer to the lash line than most devices allow.

What it does modestly: mild scarring, mild wrinkles elsewhere on the face, and skin firmness over a series. Real but not dramatic.

What it does not do: clear pigment or sun spots, lift or tighten sagging skin, or fix significant acne scars.

Timeline: a fresh look within a week of each session, cumulative improvement across three or four sessions, and continued collagen change for three to six months after the last one.

How long it lasts: roughly a year for texture and fine lines, with maintenance sessions once or twice a year for people who want to hold it.

The honest evidence position: the mechanism is sound and safety looks good, but the literature is largely small case series and retrospective reports, with very little randomized comparison against the fractional lasers Tixel is sold as an alternative to. A comprehensive review of the device's applications made exactly this point — heterogeneous reporting and few controlled trials. That does not mean it does not work. It means claims that it matches laser resurfacing with less downtime are not currently backed by the kind of evidence that would settle the question.

Who is disappointed: people with real acne scarring, people expecting laser-equivalent results, and people who bought a package of light sessions after seeing photos from aggressive ones. Also anyone who came for pigment — which Tixel cannot address at all, and which is worth confirming before you pay.

At-Home Versions

There is no home version. A 400-degree titanium array with millisecond contact control is not a consumer product, and the settings that make it safe are the whole device.

What people sometimes reach for instead is home microneedling, on the reasoning that both make tiny holes in skin. The comparison does not hold. Home rollers with short needles do very little; longer needles create real wounds without sterile technique or depth control, and the result is often worse texture than you started with. Professional microneedling is the controlled version of that idea and is a reasonable comparable clinical treatment at a similar price.

The maintenance routine matters more than any gadget: a nightly retinoid such as tretinoin or over-the-counter retinol, daily mineral sunscreen, and vitamin C in the morning. That combination improves texture and fine lines over months, holds a Tixel result far longer, and costs a fraction of a session.

FAQ+
Is Tixel a laser?No. It has no light source at all. A grid of titanium pyramids is heated to about 400 degrees Celsius and pressed briefly against the skin, transferring heat by direct contact. It is the same physics as touching something hot, controlled to a few thousandths of a second.
Then why is it on the laser menu at the clinic?Because that is where people look for resurfacing, and because the outcome it aims for — fractional injury, collagen response, smoother skin — is the same outcome fractional lasers aim for. The category is honest even though the label is not.
Does that make it safer?In one specific way, yes. Because nothing in the device targets melanin, it does not carry the pigment-targeting risk that lasers do on darker skin. That is a genuine advantage. It still delivers heat, and heat can still provoke pigment change on skin prone to it, particularly at aggressive settings.
Does it hurt?Less than most resurfacing. Each contact is a brief hot press that stops immediately, and reported pain scores are low enough that many treatments are done without numbing cream. If you are anxious, numbing is available.
How many sessions?Usually three to four, three to six weeks apart. It is a cumulative treatment.

Will it help my acne scars?Only mild ones, and only over a series. It is too gentle at usual settings for significant scarring, which needs subcision, deeper RF microneedling or ablative resurfacing.
Will it clear my sun spots?No. It has no way to find pigment. Sun spots need a light-based treatment such as IPL or a fractional laser.
Is it as good as a fractional laser?That is exactly the question the evidence does not yet answer. The published work is mostly small case series with few controlled comparisons, which is worth knowing before paying for a course. What can be said fairly is that it is gentler, less painful, cheaper, and lower risk on darker skin — and that gentler treatments generally produce smaller results.
What is the dry eye thing?A separate medical use. The device was FDA cleared in December 2024 for evaporative dry eye caused by meibomian gland dysfunction, treating the eyelids in an eye doctor's office. Unrelated to the cosmetic treatment, but it explains why the name appears in ophthalmology practices.
